『BEPPU STUDIO 01』利用申請／BEPPU STUDIO 01　Application Form

『BEPPU STUDIO 01』の利用申請フォームです。申請の際には、必ず『BEPPU STUDIO 01』利用に関する詳細 (https://www.beppuproject.com/work/4636) をご確認ください。
 なお、利用申請は下記の通り、お申込みできます。 

・長期のご利用　：使用日の12ヶ月前の月の１日から予約可能 (2025年4月に使用の場合、2024年4月1日より予約可)
・中短期のご利用：使用日の６ヶ月前の月の１日から予約可能 (2024年12月に使用の場合、2024年6月1日より予約可)

 いずれの場合も、使用日の14日前までに予約をお願いします。

This is Application form of BEPPU STUDIO 01.  When applying, please make sure to read the separate sheet 'BEPPU STUDIO 01 User Guide'.

・Long-term use: reservations can be made from the first day of the month twelve months prior to the date of use (for example, for use in April 2025, reservations can be made from 1st April 2024).
・Medium to short term use: reservations can be made from the first day of the month three months prior to the date of use (for example, for use in December 2024, reservations can be made from 1st September 2024).

In all cases, please ensure your reservation is confirmed at least 14 days prior to the date of use

利用の同意
下記の項目を確認のうえ、「同意する」にチェックを入れて申請してください。／To submit your application, please read and agree to the following terms and conditions before ticking the box □.
・利用料金およびキャンセル料を確認し、使用日の８日前までに料金を支払う。
・宗教活動、政治活動、勧誘活動または公序良俗に反する活動ではない。
・利用時間内に準備、片付け、原状回復までおこなう。
・火器を使用しない。
・備品の破損、汚損、紛失の場合は、BEPPU PROJECTに連絡をする。
・専用駐車場がないことを理解し、近隣の有料駐車場または公共交通機関を利用する。
・大きな音を伴う制作などをおこなう場合、事前に相談をする。また強い匂いを発する制作や展示・活動はおこなわない。
・利用時間内に不測の事故等が生じた場合、BEPPU PROJECTでは責任を負わない。

- The applicant must pay the fee at least 8 days before the date of use, after both the fee and cancellation fee have been confirmed.
- The use of the facility for religious, political or soliciting activities, or for activities that offend public order and morals is strictly prohibited.
- Preparation, clean-up and restoration must be carried out within the time limit of use.
- Firearms are strictly prohibited.
- In the event of damage, defacement or loss of equipment BEPPU PROJECT must be informed.
- I understand that there are no dedicated parking spaces, and to use a nearby pay car park or public transport.
- If you wish to make productions that involve loud noises, please contact BEPPU PROJECT beforehand. Do not create, exhibit or conduct activities that emit strong odours.
- BEPPU PROJECT accepts no responsibility for any unforeseen accidents that may occur during the usage hours.
